Thank you for your interest in joining Liverpool Phoenix Cycling Club.

Can you please complete this form to allow us to process your application and update our records. 

Information provided is kept on a secure database and used for club admin and emergency situations only.

Club subscriptions are £20 per year (Jan to December) and payment details will be sent to you on completion of this form 

Once the payment is made and this form is completed your membership application will be completed (subject to committee approval) and full access to club media sites will be set up.
